Smartlink’s EHR-agnostic integration technology supported Generations Family Practice’s award-recognized innovation in value-based primary care.
Cary, North Carolina — October 24, 2024 — Smartlink Health Solutions, a leading provider of interoperability and value-based care technology, announced today that its technology platform played a key role in supporting Generations Family Practice, which has been named a finalist in the Provider Sector of the 2024 Gartner® Eye on Innovation Awards for Healthcare & Life Sciences.
Generations Family Practice (GFP), a primary care organization with more than 65,000 active patients across eight North Carolina clinics, is managed by Cary Medical Management (CMM), a medical services organization that partners with Smartlink Health to streamline clinical workflows, improve care quality, and optimize value-based performance.
GFP’s recognized innovation centers on its ability to combine clinical information from the North Carolina Health Information Exchange (NC HealthConnex), multiple ACO portals, and the clinic’s EHR environment to present providers with a more complete and actionable view of patient needs.
Smartlink Health’s EHR-agnostic integration platform, combined with CMM’s operational strategy and clinical leadership, enabled an automated process for retrieving, prioritizing, and inserting relevant patient data into the clinical workflow. This collaboration helped clinics reduce administrative burden, enhance diagnostic accuracy, and close high-value care gaps.
Through this joint effort, Generations Family Practice achieved significant results in 2023, including:
- 32% reduction in total medical costs for 12,000 attributed patients across UnitedHealthcare, Aetna, and Cigna.
- Projected shared savings and incentive payments 2.5x higher than 2022, adding more than 8% to operating margin.
