Cary, NC, June 28, 2022 – Smartlink Health Solutions, an innovator in health IT integration, has been selected by Evolent Care Partners to integrate member clinic’s electronic health records (EHR’s) into their new accountable care organization, Evolent Care Partners of North Carolina. With Smartlink’s integration technology, Evolent Care Partners began receiving the required clinical data from physician practices within a couple of weeks.
The Smartlink data will supplement existing data sources that Evolent Care Partners integrates into its proprietary population health platform to strengthen and support care gap closure for over 10,000 North Carolina patients.
Smartlink’s Data Connector (SDC) is an Integration Platform as a Service that has the ability to leverage a health IT system’s user interface (UI) to exchange data. It works with any operating environment (SaaS, hosted, on-premise), and can insert or extract virtually any type of data. With SDC, integrations that often take more than six months can be completed in a matter of days or weeks and without EHR vendor involvement. For Evolent Care Partners, integrations were completed within four weeks.
